The statutory declaration required should be made before and signed by a Commissioner for Oaths at the Home Affairs Department ("HAD"), Notary Public or Justice of the Peace in Hong Kong. You may contact the Enquiry Number of the Central Telephone Enquiry Centre of HAD at 2835 2500 to check the location and office hours of the HAD district office nearest to you. Remember to bring your Hong Kong Identity Card and the relevant and completed declaration form with you.
If you are residing overseas, you may make the statutory declaration outside Hong Kong provided that the declaration is made beforehand and signed by a Notary Public or a person authorized under the law of that place to administer an oath or take a statutory declaration.
