a) Changed PS/2 processing to handle older NCR keyboards with keylocks.
         Turning keylock while terminal was off caused power up hangs.  
      b) Corrected handling when BIOS setup is password protected, plugging in
         a USB key when no additional devices attached forced BIOS setup to be
         entered without password prompt - security issue.
      c) Changed default of Port 60/64 emulation to disabled to prevent issues
         booting Windows if USB input occurs during early stages of Windows
         initialization.
